Who can request the confirmation certificate?
The confirmation certificate can be requested by anyone, provided that they know the personal data of the individual, the church in which the individual was confirmed, and provided that the latter received the sacrament of confirmation in Italy.
To request the confirmation certificate, you only need to fill out the form by entering the following information:
- name and surname of the natural person;
- date of birth of the natural person;
- place of birth of the natural person;
- name and surname of the father of the natural person;
- name and surname of the mother of the natural person;
- Church where the confirmation rite was celebrated;
- Use for which a confirmation certificate is required.
NB You can request the confirmation certificate in any church, located in the national territory, which is administered in the name of the Holy Trinity (Baptist Churches, Lutheran Churches, Methodist Churches, Waldensian Churches, Orthodox Churches and Anglican Churches/Communities).
What does the confirmation certificate contain?
The confirmation certificate contains all the data transcribed in the religious registers regarding the celebration of the sacrament of confirmation and the data of the confirmed person. In particular:
- Church where the confirmation was celebrated;
- Day and time the confirmation was celebrated;
- Full name and surname of the person who received confirmation;
- Date of birth and place of birth of the person who received confirmation;
- Name of the parents of the person who received confirmation;
- Name of the godfather or godmother of the person receiving confirmation.
- Identification number of the register of confirmed persons of the church in which the rite was celebrated.
When to request the confirmation certificate
To request a confirmation certificate it is necessary:

- to act as godfather or godmother in someone else's baptism or confirmation rite;
